What we need

We currently have a Permanent Part-Time opportunity for a skilled, enthusiastic and dedicated Home Care Care Coordinator based in our Home Care office in Kings Meadows 

As a Home Care - Care Coordinator, you will have an opportunity to:

 What you bring to the role

  • Relevant qualification in nursing, case management, community services, aged care or equivalent
  • Experience in service delivery to clients living in the community
  • High level planning and organisational skills
  • Well-developed communication skills
  • A desire to create a positive experience for our consumers
  • A current unrestricted drivers license
  • Experience in case management desirable, however training can be provided
